development of multiple implants. In these cases, postoperative radiotherapy (approximately 60 Gy) becomes necessary to destroy residual cells. Irradiation should be performed in a specialized oncology center with experience in head and neck neoplasms.<\/p><p><strong>Surgery:<\/strong> Repeat total parotidectomy with retrograde technique and neuromonitoring of the facial nerve.<\/p><p>The patient had a history of two previous surgeries to remove a pleomorphic adenoma of the left parotid gland, the last surgery having been performed 15 years ago. He now presents with extensive recurrence with multiple implantations in the central part of the parotid gland and the upper neck.<\/p><p>In the first surgery, an extracapsular resection was performed, during which a traumatic opening of the tumor occurred. In the second, a superficial parotidectomy was attempted with identification of the central branch of the facial nerve and cleaning of the implants.<\/p><p>During the third recurrence, the patient delayed treatment for approximately 10 years, due to the risk of nerve injury.<\/p><p>MRI revealed multiple implantations in the parotid and retromaxillary regions, as well as parapharyngeal extension into the deep lobe of the parotid gland.<\/p><p>It was decided to apply the retrograde technique, with the identification of peripheral branches in areas free of scar tissue. After lifting the skin flap and excising the old scar, the basic anatomical landmarks were identified: the sternocleidomastoid muscle, the angle of the mandible, the masseter muscle and the zygomatic arch.<\/p><p>With the help of neurostimulation, the five main peripheral branches of the facial nerve and the Stensen canal were identified, which were prepared towards the center of the parotid gland. Adhesions made it difficult to approach the central branch, which was completely surrounded by implants. The recurrences were removed with extreme care and the nerve, which had been displaced up to the area of \u200b\u200bthe stylomastoid foramen, was released.<\/p><p>In the deep lobe, the internal maxillary and superficial temporal arteries were located, and the parapharyngeal fat near the sphenoid process was removed. Due to infiltration of the sternocleidomastoid and the posterior stomach of the digastric, they were partially resected.<\/p><p>Lymph node dissection at levels I, II and III followed, with preservation of the internal jugular, accessory and hypoglossal nerves.<\/p><p>The operation lasted over 7 hours. At the end, the stimulation of the central branch was reduced, but most of the peripheral branches \u2013 except the labial branch \u2013 responded adequately. Postoperatively, mild paresis of the frontal branch and weakness of the labial branch were observed, with good eyelid closure.<\/p><p>\u038c\u03c0\u03c9\u03c2
